If you want to change of family physician, pharmacy or dentist there are various options. You can do this at the care provider, in the AZV app of via our website. Remember that a request for a family physician change can only take place in March and September and a request for change of pharmacy in April and October.
You can go to the care provider and make the change there. Your new doctor, pharmacy or dentist will inform AZV of the change and we will update your policy information.
You can also request a change in the AZV app. Go to ‘Menu’, click on ‘Care providers’ and after that go to ‘my care providers’ and then choose for ‘change of care provider’.
